We are very excited that you have chosen to look at Western Slope Endocrinology for your needs.
Since we are a specialty practice we do NO primary care coverage. Most often patients are referred to us by their primary care team for help with a specific endocrine disorder or set of disorders. The referral may be for advice only or for help managing a condition (see below "what type of referral do you have?"). We will see patients with a certain conditions by self-referral but these must be approved prior to scheduling. We review all referrals prior to the appointment to try to be sure that we are the right specialty for the condition and that we have the information needed to make the referral appointment meaningful. We ask that the referring practice send critical pieces of information, such as your medical history and pertinent test results, to help us make your time worthwhile. One cause of delay in scheduling is not getting this information.

We want to help you with your endocrine problems and want to help make things better for you. Please fill out the new patient forms on our patient portal (ask our front desk staff to "web enable" you at the time of scheduling your appointment. If you prefer not to use the portal then arrive early to have the medical assistant help you with the forms or to fill in paper forms.
Please bring and updated list of your medications or your medication bottles with you to each appointment. Also let us know about any supplements or other types of treatments you are getting. If you have diabetes, please bring your glucose meter or log book to each appointment.

Appointment Confirmation
We will contact you to confirm your appointment. If you do not confirm, then we reserve the right to utilize your appointment time for a patient on the wait list.
We need for you to confirm by noon the day before your scheduled appointment.
You may leave a confirmation message on our practice voicemail afterhours if that is convenient for you.
Appointment Cancellation
We ask that you notify us at least 24 hours in advance if you need to cancel your appointment.
You can leave a cancellation message on the practice voicemail after hours.
Appointment No Shows
Established patients that NO SHOW a confirmed appointment will be charged a $25 fee for the first NO SHOW appointment and $50 for a second NO SHOW appointment.
We reserve the right to dismiss you from the practice if you do not pay these fees or if you have a third NO SHOW appointment.
A NO SHOW is defined as not showing up for a confirmed appointment or a last minute cancellation of an appointment.
For a NEW patient NO SHOW appoint we reserve the right to NOT reschedule an Appointment.
What Type of referral do we have?
Consultation
The specialist will provide advice to you and your primary care physician/team or other referring physician but will not provide ongoing management or follow up of your care.
Shared Care (Co-Management)
The specialist will provide advice and some help with treatment and follow up but your primary care team or other referring physician will do the prescriptions, order any tests needed for monitoring the condition and take your calls regarding the disorder or medical condition.
Principal Care/Assumed Care (Co-Management)
The specialist will "assume care" for the disorder or medical condition and for that condition will write prescriptions, order needed testing, arrange follow up appointments and take your calls regarding the condition. This may be long-term or until the condition is improved or stabilized.
